High School Athlete of the Month Submission Form

By submitting information and/or images to Kenosha.com in conjunction with Bradford High School and Kenosha Unified School District, you hereby grant permission for the use of the submitted content across the following platforms:

Kenosha.com and its affiliated digital and social media channels

Bradford High School and Kenosha Unified School District official media outlets, including websites, newsletters, and social media platforms

This consent includes, but is not limited to, the use of submitted photograph, student responses, and related content for promotional, educational, and community engagement purposes.

You acknowledge that:

The submitted content may be edited, published, and distributed publicly

No compensation will be provided for the use of submitted materials

Kenosha.com and Bradford High School and Kenosha Unified School District retain the right to use the content in perpetuity for the purposes stated above.

By submitting, you confirm that you have the authority to grant this consent and that all individuals depicted in the submitted materials have provided appropriate permissions for public use.
